By clicking "submit", you agree to the following:

  1. That they will supply a representative from their company to perform the tasting;
  2. That the person performing the tasting will have a valid "smart serve" certificate and present same at the tasting if requested;
  3. That all the beer being dispensed at the tasting will be fully tax pre-paid;
  4. That they will provide a table to act as a counter for their tasting activities; and
  5. That they will add "La Mantia's" as a named insured under their company's general liability policy for the duration of the tasting.
